Description

• As Pair Team’s Head of Population Health you will play a critical leadership role in defining our value-based care strategy and implementing programs that improve clinical outcomes for patients and lower total costs of care. • This is a remote position reporting to the Chief Medical Officer. • While this role is listed as a "Head of Pop Health" to reflect our target experience, Pair Team will not use this title. Responsibility, ownership level, and skillset are expected to be commensurate with compensation and level.

Requirements

• 5+ years of experience in program or product management and 2+ years of experience with an early-stage startup • A track record of successfully launching clinical products/programs • Ownership mindset – own driving results for the mission, business, and customer experience • Strong collaboration skills with thought partners from every team at Pair Team • Experience conducting quantitative analysis and qualitative research (customer interviews) and to inform strategic decisions • Strong desire to work in an early stage startup environment that is fast paced, complex, and has minimal barriers
to make decisions (no “red tape”) • A “roll up your sleeves” attitude • Passion for helping individuals experiencing complex chronic needs such as homelessness, severe mental illness, and substance use disorder
