Description

• Conduct thorough patient evaluations and assessments. • Develop personalized treatment plans based on individual needs and goals. • Implement therapeutic interventions and monitor progress. • Educate patients and their families on treatment protocols and wellness strategies. • Collaborate and facilitate with other healthcare professionals to ensure coordinated care. • Maintain accurate patient records and documentation. • Attend meetings and case conferences • Communicate daily with office staff to accept new patients, complete paperwork and turn in payroll • Completion of Evaluation, Plan of Care, Reassessment, and Discharge visits and paperwork.

Requirements

• Current Physical Therapist Licensure • Ability to travel to patient’s home or facility • Strong communication skills • Home healthcare experience preferred
