📋 Description
• Evaluates patient’s functional status and home environment for hazards. • Identifies equipment needs and participates in care plan development. • Communicates the plan of care to referring physician and other professionals. • Trains patients in the use of adaptive equipment and maintains clinical records. • Attends meetings to ensure coordinated care for patients. • Instructs families/caregivers in treatment regime when indicated and appropriate.
