Description

• As a Physical Therapist (PT), you are key to helping us provide superior patient care • You will work to provide skilled Physical Therapy services in accordance with the client’s plan of care • Help clients to relieve pain, develop or restore functional and maintain maximum performance, using physical means • Provide initial and ongoing comprehensive assessments of the client’s needs • Establish a treatment plan in consultation with the physician • Assist in development and implementation of the interdisciplinary client care plan • Treat clients to relieve pain, develop or restore functional and maintain maximum performance • Consult and familiarize staff regarding the established plan • Supervise Physical Therapy Assistants and paraprofessionals as per agency policy • Communicate with the physician at least every 30 days or whenever changes occur

Requirements

• Must be a graduate of a Physical Therapy school approved by the Council of Medical Education of the A.M.A. in collaboration with the American Physical Therapy Association • Must be licensed or registered in Virginia • Must have at least one year of institutional/home health experience • Must have current CPR certification • Working knowledge of Medicare regulations and guidelines is a plus • You must have a minimum of one year experience as an PT, home health experience is strongly preferred • Must be proficient using PT equipment • You must be able to provide two references • You must be able to pass OIG checks, Virginia State Police background check, and national background checks, and able to verify employment eligibility with necessary documentation • Reliable transportation, proof of valid car insurance, and a valid driver’s license
