📋 Description

• Collaborates with care team to improve quality of care for members • Coordinates assessments, planning, and communication to prioritize member needs • Engages members through strength-based assessments • Develops and updates individual care plans in collaboration with the care team • Identifies barriers and creates interventions to meet care plan goals • Educates members about treatment options and resources • Monitors member satisfaction and healthcare resource utilization

🎯 Requirements

• Nursing degree from an accredited nursing program or Bachelor’s degree in a health care field or equivalent years of relevant work experience is required • A minimum of three (3) years of experience in nursing or social work or counseling or health care profession is required • Three (3) years Medicaid and/or Medicare managed care experience is preferred • Current unrestricted clinical license in state of practice as a Registered Nurse, Social Worker or Professional Clinical Counselor is required • Case Management Certification is highly preferred
