Clinical Pharmacist - Care & Medication Therapy Management

Description
• Manage populations of patients who meet high risk criteria • Ensure safe, effective, and cost-effective medication therapy • Utilize client formulary information and member specific benefits • Be aware of current and emerging clinical trends • Collaborate with physicians and other prescribers • Responsible for performing comprehensive and targeted medication reviews • Support quality department to improve HEDIS metrics
Requirements
• Bachelor’s degree or advanced degree (Pharm.D., M.S.) in pharmacy • 2+ years of mail order, retail, hospital, managed care pharmacy experience or pharmacy residency program experience • Pharmacist must be licensed in Missouri and also needs to reside in Missouri • Highly preferred: Experience in behavioral health
