• Help elevate patient care to a new level! • Join Aetna as an industry leader in serving dual eligible populations utilizing best-in-class operating and clinical models. • Have life-changing impact on DSNP members with complex health and social challenges. • Collaborate with members, providers, and community organizations addressing health care and social determinant needs. • Utilize critical thinking and judgment to inform the case management process. • Facilitate healthcare outcomes for members through appointment scheduling and accessing benefits. • Conduct comprehensive evaluations of referred member needs/eligibility. • Coordinate care plan activities and monitor progress. • Consult with health program stakeholders to achieve optimal outcomes. • Ensure maximum member engagement and promote lifestyle changes for optimum health.

🎯 Requirements

• 2 years experience in case management, • behavioral health, • social services or appropriate related field equivalent to program focus, • Discharge planning experience (preferred), • Managed care experience (preferred), • Bilingual (English/Spanish) (preferred), • Excellent analytical and problem-solving skills, • Effective communications, organizational, and interpersonal skills. • Ability to work independently, • Effective computer skills including navigating multiple systems and keyboarding, • Proficiency with standard corporate software applications, including MS Word, Excel, Outlook, and PowerPoint, as well as some special proprietary applications.
