Description

• Provide information, referral, and health coaching to Medicare members. • Instruct group coaching classes on chronic conditions. • Educate members on Managing Medicines, Nutrition, Safety, Benefits/Resources, Mindfulness. • Assist members in participating with their healthcare providers. • Document member interactions appropriately. • Troubleshoot unavailability of resources and secure support. • Follow program workflows and review personal action plans with members.

Requirements

• At least 2 years working as a Health Coach. • Experience in care management, healthcare, social work, or aging network preferred. • Community health or public health experience preferred. • Managed Care/Medicare/Medicaid experience preferred.
