• At Diverge Health we are a team of entrepreneurs passionate about improving health access and outcomes for those most in need. • We partner with primary care providers to improve the engagement and management of their Medicaid patients, providing independent practices access to specialized resources and clinical programs to address medical, social and behavioral patient needs. • Our care ecosystem is equipped with enhanced technology and data interfaces to enable provider and patient success in a value-based environment. • Guided by our core values of humility, continuous learning and feeling the weight, our team is on a mission to strengthen communities from within, unlocking people's ability to live their healthiest lives. • Reporting to the Chief Medical Officer, the Program Manager will be a key member of the Population Health team. • We are looking for someone with strong project management experience that is excited about working in a fast-paced environment. • As someone with experience in a service-based business that has grown to multiple markets, the Program Manager will work to manage our clinical program development and deployment across markets. • As a natural organizer, you will keep the Population Health team on track through monitoring of KPIs, tracking team OKRs, and driving projects to completion through cross functional collaboration. • Additionally, you’ll stay up to date on industry trends, and work to create a connected/collaborative culture by creatively sharing insights and lessons learned from the field as we continue to refine our care model. • As someone willing to take on tasks of varying complexity, you may sometimes organize one-off in-person meetings for the Population Health team.

Requirements

• 5+ years of project management for clinical programs in a health system, value-based care organization, or similar business • Operations experience with a large enterprise. • Expert in Microsoft Office, Microsoft PowerPoint, Microsoft Excel • Excellent written and verbal communication skills • Ability to communicate to diverse audiences • Ability to manage up and assist leaders in prioritization and execution of tasks • PMP certification strongly preferred • Experience working in a value-based care organization • Experience with agile methods
