Description

• Conduct UAS assessments at intervals required by the Department of Health (DOH) to determine service needs and continued eligibility • Coordinate with Care Management Team to maintain a high level of efficient and effective individualized Care Management operations • Provide weekly work availability to our UAS coordinator • Notify our UAS coordinator promptly of scheduling delays or conflicts • Schedule appointments within 24 hours of case assignment • The UAS assessment process must be completed within 7 days of the case assignment • After finishing an assessment, the case must be transmitted and related paperwork must be completed and submitted within 24 hours • Conduct quality assurance checks on all completed documents before finalizing and sending them • Abide by and promote our agency’s administrative policies • Attend meetings and training as required • Maintain current and active Health Commerce System access • Maintain a continuing knowledge of competencies related to the nursing profession by participating in formal education programs, conferences, workshops, and professional organizations • All consent and original intake assessment documents must be submitted within 5 days via mail or dropped off at a health home location • Original vouchers for payment must be submitted bi-weekly

Requirements

• Graduate of 2 - 4 year accredited Nursing program • A bachelor’s Degree in Nursing or an advanced degree is preferred • Active and unrestricted NY State Registered Nurse (RN) License • CCM Certification • Knowledge of current community health practices for the frail adult population, behavioral health, and cognitively impaired seniors and disabled including knowledge of physical health, aging, and loss, appropriate support services in the community, frequently used medications and their potential negative side effects, depression, challenging behaviors, Alzheimer’s disease, and other disease-related dementias • Minimum of three to five years of demonstrated clinical and supervisory experience in an MLTC setting • Proven knowledge of UAS-NY Assessments • Knowledge of MLTC regulatory requirements • Knowledge of current community health practices and resources available • Solid assessment and documentation skills • Ability to manage difficult peer situations arising from medical care review • Ability to effectively communicate verbally and in written formats at all levels • High level of professionalism with the ability to handle sensitive and confidential information • Meticulous attention to detail and strong organizational skills • Ability to use computerized systems such as Microsoft Outlook, Microsoft Word, and Excel • Travel by public transportation only in Manhattan all other cities require transportation via private vehicle
