📋 Description
• Lead a community-based team of Care Extenders for program delivery. • Collaborate with members for personalized case management targeting specific needs. • Create service plans based on comprehensive member information. • Educate and encourage members on mental and physical health strategies. • Monitor and evaluate member progress toward their goals. • Build communications to enhance team member engagement. • Conduct hiring, training, and performance reviews for new team members. • Ensure team meets Key Performance Indicators and manage caseloads. • Analyze performance to identify challenges and improve processes. • Participate in external meetings with health plan partners.
🎯 Requirements
• A Qualified Mental Health Professional (QMHP) holding one of the following licenses, preferably in Louisiana: Licensed Clinical Social Worker (LCSW) • Clinical Professional Counselor (CPC) or Licensed Professional Counselor (LPC) • Licensed Marriage and Family Therapist (LMFT) • Licensed Psychologist (PhD or PsyD), or equivalent • If license is not in Louisiana, it needs to be in one of the following states where this program is active: Indiana, Nevada, Tennessee, Texas, or Washington. • Experience leading, building and implementing programs and delivering outcomes. • Strong experience leading a team. • Experience with Health Coaching or Medical Social Work preferred. • Master's degree in behavioral health, social work or health care management from an accredited university required.
