Description
• Assess, plan, and implement care strategies individualized by member • Collaborate with member, family, and healthcare providers • Conduct individual counseling and group therapy • Identify and initiate referrals for social service programs • Advocate for members and families as needed • Utilize approved clinical criteria to assess and determine appropriate level of care • Document all member assessments and care plans • Participate in interdisciplinary team meetings • Maintain excellent punctuality and attendance
Requirements
• LCSW, LMFT, LMHC, LPC in KY, IN, or TX State • Preferred experience with substance abuse population • Background in maternal; substance abuse preferred • Experience with adults and adolescents • Familiarity with Medicare and Medicaid procedures • Remote Work Experience
