Description
• This is a remote position for casual coverage. • Acute Care Management Social Worker experience preferred. • Responsible for providing services to patients, families, and significant others. • Completion of biopsychosocial assessments and psychiatric diagnoses. • Formulation of treatment plans and treatment interventions. • Conducting therapy sessions and discharge planning. • Collaborates with interdisciplinary team members for referrals.
Requirements
• Master's Degree (Required) • Valid state driver’s license, if in Home Care department • Minimum of eighteen (18) years old • Acceptable Motor Vehicle Report (MVR) • Proof of insurance, if applicable. • Interview, assessment, organizational and problem solving skills. • Ability to identify appropriate community resources on assigned caseload. • Work collaboratively with patients, families, multi-disciplinary team and community agencies. • Demonstrates patience and tact when dealing with patients, families and other staff. • Promotes quality patient care and customer service/satisfaction.
