📋 Description
• Offer telephonic case management for members with severe or chronic health conditions • Coordinate care and develop patient treatment plans • Telephonically manage cases per established guidelines • Complete thorough assessments of patients' needs • Develop treatment plans with patients and caregivers • Monitor interventions and report effectiveness • Maintain contact with patients and multidisciplinary teams • Advocate for quality patient care and support • Meet daily productivity requirements • Negotiate and implement cost management strategies • Ensure confidentiality according to company policy
🎯 Requirements
• Current unrestricted RN license • California license (preferred) • 1+ years of case management experience required • Computer proficiency (Microsoft Office) • Knowledge of medical claims and coding preferred • Knowledge of utilization management case philosophies preferred
