📋 Description
• Seeking a compassionate Patient Care Manager for managing chronic diseases. • Conduct comprehensive patient assessments and develop care plans. • Educate patients on managing chronic diseases and coordinate care. • Monitor data from RPM for trends and report findings. • Document patient interactions accurately in EHR systems.
🎯 Requirements
• Certified Medical Assistant (MA) or Licensed Practical Nurse (LPN) credential required. • Certification in disease management, case management, or patient education is a plus. • At least 2 years of experience in patient care, care coordination, or chronic disease management. • Experience with CCM or RPM programs strongly preferred. • Proficiency in electronic health records (EHR) systems. • Experience with Google Workspace. • Familiarity with RPM devices and data monitoring tools is an advantage. • Excellent interpersonal and communication skills for patient education and team collaboration. • Strong organizational and multitasking abilities with attention to detail. • Reliable internet connection and a HIPAA-compliant workspace for telephonic and virtual interactions.
