📋 Description

• Provides comprehensive assessment, diagnosis and treatment of mental, emotional, behavioral, addictive and developmental disorders and disabilities using specialized clinical knowledge and advanced clinical skills. • Provides comprehensive biopsychosocial and standardized assessments using specialized clinical knowledge and advanced clinical skills for potential program enrollment. • Works with the patient to develop an initial individualized support plan with goals. • Provides resources for services in the community and makes referrals. • Provides enrollment and intake services to patients and potential patients. • Responds to telephone inquiries from persons seeking mental health care or information about mental health related services. • Conducts safety assessment and planning with patients via telephone or video. • Partners with patients to create self-directed sustainable changes that align with their values to promote health/wellness and enhance well-being. • Collaborates with other members of the multidisciplinary team to develop support goals for the patient. • May participate in multidisciplinary patient care conferences discussing information obtained from patient assessments, updates, and plans. • Completes necessary documentation and reporting in accordance with departmental standards and federal and state regulations.

🎯 Requirements

• Master's degree in Behavioral Health or education equivalency for licensing • Licensed Clinical Professional Counselor (LCPC) in Illinois or Licensed Clinical Social Worker (LCSW) in Illinois or Licensed Marital and Family Therapist (LMFT) in Illinois • Licensed Clinical Social Worker (LCSW) in Missouri or Licensed Marital and Family Therapist (LMFT) in Missouri or Licensed Professional Counselor (LPC) in Missouri • Licensed Clinical Social Worker (LCSW) in Oklahoma or Licensed Marital and Family Therapist (LMFT) in Oklahoma or Licensed Professional Counselor (LPC) in Oklahoma • Licensed Clinical Social Worker (LCSW) in Wisconsin or Licensed Marital and Family Therapist (LMFT) in Wisconsin or Licensed Professional Counselor (LPC) in Wisconsin
