Description
• Promote improvement of health outcomes • Assess, plan, implement, coordinate care • Manage precert and concurrent reviews for transplant members • Establish collaborative relationships with clients and providers • Implement and evaluate case management plans
Requirements
• Active and unrestricted Registered Nurse (RN) license in state or territory of the United States • Two years full-time equivalent of direct clinical care to the consumer • Case Management experience strongly preferred • Medicare experience strongly preferred • CCM Certification preferred
