📋 Description
• Company Overview: Wider Circle is dedicated to improving health outcomes through community-driven care programs that connect neighbors to support and motivate one another. Partnering with health plans and providers nationwide. • Position Summary: The Community Health Worker (CHW) – Outreach Specialist will play a critical role in connecting members with Wider Circle’s programs through remote outreach operations. • Key Responsibilities: Conduct outbound calls to engage members, manage inbound calls, provide program information, execute targeted outreach campaigns, and collaborate with team to refine outreach strategies.
🎯 Requirements
• Education: A high school diploma or equivalent is required; additional certification as a community health worker • Experience: certification as a community health worker or 1-3 years of relevant experience in community health, social services, or related fields as it is directly related to the field of Community Health Work. • Bilingual: English and Spanish • Must reside in the state of CA
