📋 Description
• Providing individualized support within the client’s personal home and surrounding community, following the client’s treatment plan outlined and created by the client’s family, clinician, and case management team. • Identifying the needs of qualified clients, and the services necessary to meet client needs and facilitate access to those services and supports. • Conducting intake/assessment, plan of care development, coordination/advocacy, monitoring, and evaluation. • Developing and implementing a family strengths-based model utilizing the wraparound process and the service plan in accordance with the client/team/supervisory input. • Completing session notes and related documentation in the EQ Maine Health Care Management system.
🎯 Requirements
• A bachelor’s degree in psychology, or a closely related field. • One year of experience directly related to the human service field. • An active BHP certification or willingness to enroll in and complete BHP training within 1 year of hire. • Current Maine Driver’s License, registered, insured, and inspected vehicle. • Motivated and able to work independently and in coordination with others.
