📋 Description

• Coordinate patient care by reaching out to new referrals and scheduling/rescheduling appointments. • Serve as the first point of contact for patient inquiries, addressing or escalating questions as needed. • Monitor and respond to patient messages, ensuring timely follow-up if physical therapists are unavailable. • Lead and manage group sessions for patient support and education. • Assist with asynchronous patient care activities using the BreatheSuite dashboard. • Provide coverage and support for physical therapists during time off or sick leave. • Facilitate initial PTA led onboarding sessions with recently evaluated patients to introduce them to the pulmonary rehab program and their PT prescribed exercise prescription. • Perform clinical operations support tasks as required to enhance the efficiency of the rehab program.

🎯 Requirements

• Prior experience in patient care coordination or clinical support (virtual healthcare experience is a plus). • Strong communication and organizational skills. • Ability to work independently in a remote setting and collaborate with a virtual team. • Familiarity with patient management software or dashboards is preferred. • Compassionate and patient-centered approach to care.
