• The CareBridge Community and Behavioral Navigator provides telehealth services to managed LTSS patients. • Assist in holistic assessment, planning, coordinating, and monitoring healthcare access. • Facilitate coordination of services/resources for members. • Support implementation of interventions per the Person-Centered Support Plan (PCSP). • Respond promptly to CareBridge team referrals and participate in on-call emergencies. • Complete psychosocial and economic or SDOH assessments of patients. • Communicate effectively during telephonic communications. • Collaborate with healthcare providers to coordinate member care. • Assess and monitor member progress toward goals. • Collaborate with community outreach advocates for member coordination. • Develop goals and interventions with patients, families, and care coordinators. • Deliver targeted interventions for identified patients. • Facilitate Advance Care Planning discussions. • Provide education on disease state and available resources. • Use motivational interviewing to address patient concerns. • Assist with maintaining medical appointments and connect members to resources. • Document assessments in the CareBridge system. • Maintain excellent punctuality and attendance. • Perform other duties as assigned. • Potential for up to 20% travel as needed.
• 3+ years’ experience of social service or community outreach experience • Experience working with adult chronic disease patients and geriatric and IDD population • Works and communicates well in a team environment • Maintains a patient-centered focus • Excellent communication skills • Basic computer skills and understanding • High School Diploma is required • Bachelor’s degree in Social Work, Health Services, Behavioral Science, or related field (preferred) • Medicare and Medicaid experience preferred (preferred) • Bilingual English/Spanish language skills (preferred)
