Description

• The CareBridge Community and Behavioral Navigator provides telehealth services to managed Long Term Support Services patients. • Assists in holistic assessment and care coordination for healthcare services access. • Collaborates with interdisciplinary teams for member care and support. • Provides patient education, coaching, and compliance assistance. • Coordinates community resources and services for members. • Participates in case discussions and documents care in the medical record system. • Potential for up to 20% travel to meet patient needs.

Requirements

• 3+ years’ experience of social service or community outreach experience • Experience working with adult chronic disease patients and geriatric and IDD population • Works and communicates well in a team environment • Maintains a patient-centered focus • Excellent communication skills • Basic computer skills and understanding • High School Diploma is required • Bachelor’s degree in Social Work, Health Services, Behavioral Science, or related field (preferred) • Medicare and Medicaid experience preferred (preferred) • Bilingual English/Spanish language skills (preferred)
