• Supports community connection activities • Provides members with known community resources • Supports the care team to identify member community support • Provides health education as appropriate • Supports all member related correspondence • Conducts non-clinical general health assessments • Conducts telephonic and/or in-person outreach • May make visits to individual homes and/or community organizations • Working Knowledge of Social Determinants of Health (SDOH) barriers

Requirements

• Requires a High School diploma or GED • Requires 1 – 2 years of related experience
