• Looking for dedicated Home Health Field Physical Therapist Assistants for My Choice Therapy. • Must be able to cover Palmdale & Lancaster cities (REQUIRED). • This is a 1099 Contractor Position Only (Must have Business License or Sole Proprietorship). • Paid Per Visit - Flexibility to create your own schedule.
Requirements
• A Valid Practicing License from the State of California • Business License or Claim Sole Proprietorship • Professional Liability Insurance • CPR Certification • Driver’s License • Auto Insurance • Physical Exam (within 6 months from hire) • TB Clearance (within 6 months from hire)
