• Critical role within the DSNP team • Outreach to DSNP members via phone to introduce services • Complete the Health Risk Assessment (HRA) • Build trusting relationships with members • Support members during enrollment calls • Document calls in the member’s electronic record • Protect confidentiality of member information

Requirements

• 3 years of experience in health care, customer service, telemarketing and/or sales • Familiarity with basic medical terminology, health care, and care management concepts • Experience with computers and knowledge of Microsoft Word, Outlook, and Excel • Flexibility with work schedule to meet business needs • Preferred: Medicare/Medicaid/DSNP experience • Strong organizational skills, including verbal and written communication • Preferred: Data entry and documentation within member records • Preferred: Bilingual Spanish • Preferred: Associates and/or Bachelor’s Degree
