Requirements
• Assist individuals with disabilities with community activities, • Follow individuals’ support plans, • Administer medications according to guidelines, • Encourage community participation, • Document all observations and daily progress notes, • Communicate effectively with all team members, • Schedule, transport, and accompany individuals to appointments, • Provide premier personal care, • Act as a positive role model, • Meet all training requirements for active roles, • Perform additional responsibilities as assigned by supervisor
