β’ RN, Field Care Managers perform face-to-face and telephonic assessments to adult and pediatric members. β’ Evaluate member's needs to achieve and/or maintain optimal wellness. β’ Manage case load and complete assessments with members in home or community-based settings. β’ Provide clinical support and guidance, particularly for members with medical complexity. β’ Develop and modify Individual Care Plans and coordinate care plans ensuring appropriate services.
β’ Must reside in the Commonwealth of Virginia or within a 20-mile radius from Virginia in a bordering state/district. β’ Active Registered Nurse (RN) license in the Commonwealth of Virginia without disciplinary action. β’ Two (2) years of prior experience in health care and/or case management. β’ One (1) year of experience working directly with individuals who meet the Cardinal Care Priority Population criteria. β’ Strong advocate and respect for members at all levels of care. β’ Intermediate to advanced computer skills and experience with Microsoft Word, Excel, and Outlook. β’ Exceptional oral and written communication and interpersonal skills. β’ Ability to work with minimal supervision within the role and scope. β’ Valid state driver's license and maintain personal vehicle liability insurance.
