📋 Description
• Contribute to and execute clinical consulting engagements by assessing workflows, identifying inefficiencies, and designing creative, evidence-informed solutions using the ThoroughCare platform • Guide clients in implementing and optimizing care management programs (CCM, RPM, BHI, TCM, etc.) • Assist in developing clinical best practices, documentation guidelines, and internal process improvements • Collaborate with the product team to provide clinical insights and feedback during feature development and testing • Support training and education for clients around documentation quality, care planning, time tracking, and patient engagement • Analyze program performance data and contribute to recommendations that improve efficiency, compliance, and outcomes • Participate in cross-functional initiatives that support clinical strategy and long-term client success
🎯 Requirements
• Bachelor’s degree required (nursing, public health, health informatics, or related field) • Active clinical license (RN, LCSW, OT, PT, or similar) • 2+ years of relevant experience in value-based care models • Familiarity with CMS care management programs, including CCM, RPM, BHI, TCM, and AWV. • Demonstrated experience leading or supporting quality improvement initiatives such as chart audits, training initiatives etc. • A creative, solution-focused mindset with the ability to bridge strategic goals with real-world clinical workflows • Strong communication and interpersonal skills with confidence working in a collaborative, client-facing environment • Highly organized and self-motivated, with the ability to thrive independently in a fully remote environment
