Description

• Serve as physician champion/liaison for our remote cardiovascular risk prevention program for the region • Review and approve Individual Treatment Plans (ITPs) for patients in our cardiovascular risk prevention program • Work with our clinical team to ensure that all patients are being appropriately treated and triaged as needed • Communicate and Interface with referring physicians about individual patient outcomes and progress toward goals • Engage leadership at health systems, health plans, department of health, physician practices, Cath labs on the need for alternate delivery models for cardiac rehab and educate them on the benefits of the Movn program • Work closely with cardiology practices, Cath labs and health systems to streamline their cardiac rehab referral processes and enroll patients in the Movn program • Help the Movn team solve problems and communicate with referring health care practitioners and other stakeholders

Requirements

• Medical degree, either an M.D. or D.O. • Board certified in Cardiovascular Disease or Internal Medicine • Licensure in good standing to practice medicine in state of residence • Training in basic life support or advanced cardiac life support • Poses all necessary insurance including medical malpractice and liability • Credentialing - You will be credentialed to be a provider under the Movn Health Medical group • Privileges - Possess hospital / admitting privileges at a local hospital in your region • Private Medical Office Address: Possession of a private medical office address in your region
