📋 Description

• Manage a caseload of complex pediatric members • Conduct member visits including home visits • Perform new member visit and ongoing assessments • Develop and maintain Person Centered Support Plans • Collaborate with private insurance and Medicaid to coordinate care • Support utilization management functions for complex cases • Serve as liaison between members, providers, and customers • Oversee highly complex cases to ensure effective implementation • Perform essential activities of case management

🎯 Requirements

• License: Registered Nurse (RN) with active license in Tennessee or participating in Nurse Licensure Compact Law • 3 years clinical experience required • 5 years experience in health care industry • 2 years experience in IDD for Select Community required • Certified Case Manager (CCM) credential or must obtain within 2 years of hire • Certification in Developmental Disabilities Nursing (CDDN) required at hire or within 3 years • Excellent oral and written communication skills • Basic Microsoft Office and E-Mail skills required
