• Provide medication management services for the Virtual Pharmacist Care Center (VPCC) • Engage in Medication Therapy Management (MTM) and Comprehensive Medication Management (CMM) activities • Conduct assessments and evaluations of medication therapy for chronic conditions
Requirements
• Current CA Registered Pharmacist License • Comfortable communicating with patients via telephone and/or video technology • Proficient in basic Microsoft Office applications including Word, Excel
