β’ Beatrice Loving Heart is seeking a qualified, experienced Supervisor for the Team Manager position. β’ The Team Manager will assist our clients in the state of Maryland. β’ This position is remote, and Zoom will provide training during the COVID-19 crisis. β’ The Team Manager works interdependently with the assigned team to maximize individual quality outcomes. β’ Tasks the Team Managers perform include but are not limited to assessing clients' needs, situations, strengths, and support networks to determine their goals. β’ Responsibility includes direct supervision of assigned supervisors, conducting staff performance evaluation, enforcing policies and all Agencys operational procedures, recommending disciplinary actions, monitoring Agency reports, and ensuring timely submission of essential reports. β’ Ensure that Team Members are performing at an optimal level. β’ Coordinate and work with the Training Team to facilitate the training of new hires. β’ Evaluate the client's capacities to assist in developing plans of service for coordination services. β’ The qualified applicant will be able to promote positive health behaviors to support optimal health and well-being. β’ Ability to Telework from home. β’ Must have a high-speed Internet connection and familiarity with Microsoft Office Suite (Word, Excel, Access, and Outlook) databases. β’ Establish telephone contact with all clients, family members, and caregivers to assist with the coordination of services while monitoring the existing plans of service. β’ Willingness to provide on-site visits to Individuals and coordinate a person-centered Plan of Service. β’ The qualified applicant will be able to gather information to assist in the development of plans of service provided to clients. β’ The qualified candidate will be able to manage multiple priorities and tasks with the flexibility to improve services rendered to clients. β’ Uphold the organizations contractual billing guidelines when documenting activities in the system. β’ Collaborate with Social Workers, Registered Nurses, and others to assist Individuals with complex medical/psychological needs. β’ Educate clients on self-direction, its philosophy, and its purpose, and assist with accessing resources. β’ Implement critical thinking in assessing and resolving complex client-related crises. β’ Assist clients with transitions from nursing or assisted living facilities to independence within the community. β’ Access internal and external resources for clients and collaborate with local community agencies and providers to achieve optimal client-desired outcome measures.
β’ The candidate will possess a Masters degree in health/human services or a related field (psychology, sociology, counseling, physical therapy, social work, and any relevant field). β’ Minimum of three years experience in case management. β’ Must possess oral and written communication skills. β’ Ability to research a computer and other sources. β’ Ability to use Microsoft Office, Excel, and PowerPoint. β’ Ability to write routine reports and correspondence. β’ Ability to communicate effectively with individuals with varied cognitive abilities to establish professional relationships. β’ Ability to read and interpret documents such as employment paperwork, assessment reports, and procedure manuals. β’ Must possess a valid Maryland driver's license and reliable transportation. β’ Ability to work with little supervision, manage schedule, prioritize, and perform multiple tasks effectively.
