• My Choice Therapy is a growing Home Health Therapy company providing quality services to patients in the comfort of their homes. • This is a position where you are driving to see patients in their home. • Flexible Schedule - As an independent contractor, you can choose your patient volume and schedule patients on days and times that work with your schedule. • Per Diem Employee Status Only – Paid Per Visit - You have the flexibility to create your own schedule by providing your available days and areas of coverage.
Requirements
• A Valid Practicing License from the State of California • Business License or Claim Sole Proprietorship • Professional Liability Insurance • CPR Certification • Driver’s License • Auto Insurance • Physical Exam (within 6 months from hire) • TB Clearance (within 6 months from hire) • 1 year of home health experience as a PT
