• Care Manager responsible for managing care for low-risk members. • Interact with members via phone to coordinate care. • Create and maintain individualized care plans. • Participate in interdisciplinary team meetings. • Identify members needing clinician involvement and transition care appropriately.

Requirements

• 3+ years’ experience in health-related field. • 2+ years of customer service experience. • 2+ years of experience with claims. • Familiar with community resources & services. • Strong organizational skills. • Works independently. • Proficient with computer systems. • Knowledgeable in Microsoft Office Software. • Excellent customer service skills. • Bilingual (English/Spanish; English/Creole) preferred.
