Description

• The Community Support Worker (CSW) is responsible for conducting patient support activities in accordance with District Health Care Services and Department of Behavioral Health requirements. • Provide consumer services that are essential for assisting the consumer in the achievement of rehabilitation and recovery goals as defined in the consumer’s Treatment Plan. • The CSW is engaged in activities in the community and/or in the consumer’s natural setting 90 percent of the time.

Requirements

• Associates Degree or three years’ experience in a relevant human service field • One year experience of human service activities • Strong customer service background • Knowledge of medical terminology and insurances • Good oral, written, and telephone communication skills • Ability to work independently or in a team-oriented environment and interrelate well with individuals with diverse ethnic and cultural backgrounds and needs. • Basic working knowledge of computers
