📋 Description

• Responsible for leading projects that improve workflows and/or address issues in the daily operations of the Population Health division. • Represents the division with both internal and external stakeholders in developing programs, resolving issues and proactively researching workflows to improve operations and service delivery. • Responsibilities include implementation of programs; acting as an expert resource; mentoring and training staff; taking a leadership role in activities that contribute to meeting the mission, goals and service expectations of Population Health. • Also responsible for providing information and developing protocols to assist with meeting the requirements of accrediting agencies. • Provides input to the leadership team regarding protocols, staffing, policies, procedures, systems and operations.

🎯 Requirements

• Bachelor’s degree in healthcare or related field • At least 3 years of project management experience • Project Management Professional Certification or other project management experience including SCRUM or Agile • Experience leading and implementing organization change, setting performance measurements, establishing plans and monitoring processes, program development and process
