📋 Description

• As Program Manager, you will manage a team dedicated to providing longitudinal care management for patients with kidney disease. • You will serve as a collaborative and supportive leader responsible for the day-to-day operations of your care management team, focusing on proactive, person-centered care. • Your primary duties include managing the performance of team members, providing ongoing training and development, and ensuring adherence to best practices and protocols. • Programs at Evergreen include Chronic Complex Care Management, Compassionate Care Management, Post Acute Care, Transitions of Care, and CKD Management. • While our Program Manager positions are fully remote, each role does have a focus within specific time zones. 🎯 Requirements

• Bachelor’s degree in healthcare or nursing (BSN required) • You have an active state nursing license in good standing • Minimum of 3 years of longitudinal clinical nursing care management experience required • Minimum of 2 years' experience in a clinical nursing managerial or leadership role • Experience in longitudinal care management • Experience in Value Based Care • Business continuity planning; building resiliency into business processes / systems • Team and talent development • Relationship, partnership building, communication, change management and influencing skills • The ability to work effectively both independently and as part of a team, in collaborative settings • Maintaining high levels of confidentiality • Knowledge of value-based healthcare operations in disease management and population health programs • Intermediate skills with MS Office Suite of products including Outlook and Teams • Able to work effectively in a primarily remote environment: • Home internet must support a minimum download speed of 25 Mbps and upload speed of 10 Mbps. Cable, Fiber, or DSL connections hardwired to the internet device are recommended • Evergreen will provide remote employees with telephony applications and equipment to meet the business requirements for their role • Employees must work from a dedicated space lacking ongoing interruptions to protect member PHI / HIPAA information
