Description

• Conduct in-person and telephonic non-clinical assessments • Work collaboratively with Humana associates • Assist enrollees with scheduling health visits • Assess enrollee barriers to healthy living • Act as an enrollee advocate with providers and community resources • Monitor enrollee compliance with their care plan • Support enrollee self-management with health education • Attend community events to connect with enrollees • Implement community assessments to identify resource gaps

Requirements

• Must reside in Kentucky • 2+ years of professional human services experience or recent service in the same role • Proficient in Microsoft Office (Excel, Word) • Experience using a variety of electronic information applications/software programs • Required to be screened for TB • Requires a valid state driver's license and proof of personal vehicle liability insurance
