Description

• Conduct in-person and telephonic non-clinical assessment(s) to understand enrollee care needs, preferences, and socioeconomic barriers • Work collaboratively with other Humana associates • Assist enrollees with scheduling physical health and behavioral health (BH) office visits • Assess enrollee barriers to healthy living • Act as an enrollee advocate with providers, community resources, schools, and others • Monitor enrollee compliance with their care plan • Provide social support to help boost enrollees’ morale • Support enrollee self-management through culturally appropriate health education and health coaching • Serve as a key knowledge source for community services and information • Build and maintain relationships with providers and community resources

Requirements

• Must Reside in Kentucky • 2+ years of professional human services experience OR recent service in the same role, working in-person with consumers conducting assessments and/or developing care plans • Proficient in Microsoft Office (Excel, Word) • Experience using a variety of electronic information applications/software programs • This role is considered patient facing • Requires an individual to have a valid state driver's license and proof of personal vehicle liability insurance with at least 100,000/300,000/100,000 limits • Bachelor’s degree in social work or health related services (preferred) • Community Health Worker Certification (preferred) • Bilingual English/Spanish: ability to read, write, speak fluently without limitations or assistance (preferred) • Previous experience working in the community (preferred) • Experience navigating housing needs for at-risk populations (preferred) • Home Health experience (preferred)
