• About Us • One Medical is a primary care solution challenging the industry status quo by making quality care more affordable, accessible and enjoyable. • The Virtual Nurse Practitioner will care for geriatric, frail, and clinically complex patients through a team partnership with the High Risk Care Medical Director and high risk teams. • Live our mission of “restoring humanity to healthcare”, for patients and for yourself. • Rediscover the joy in practice while working in a value-based care environment with a team to support you in delivering high quality patient care. • Independently manage patients with multiple, complex conditions through a balance of tele-assist home visits (video visits with field care coordinators in the home) and telephone/video interventions. • Complete thorough initial visits with patients, diving deep into medical, social and behavioral context. • Complete ongoing follow up visits with patients, focusing on geriatrics and chronic condition management. • Work directly with the patient and through the multidisciplinary care team to impact ongoing care with particular emphasis on preventable hospitalizations and ensuring care aligned with patient goals.

Requirements

• Completed an accredited NP program with a national certification (ANCC, AANP) in Family, Geriatrics, or Adult • 5+ years of experience as a Nurse Practitioner with at least 2 years of home based, in office or virtual complex care, Geriatric Primary Care, geriatrics or palliative care experience • Bilingual in Spanish and English highly preferred • Currently licensed in Arizona and able/ willing to obtain licensure in the state of Washington and additional States as requested • Must be based in PST time zone • Must be willing to flex between PT/MT with advanced notice • Experience with home-based care preferred, specifically in the Seattle metropolitan area • Experience with value-based care principles and risk adjustment • The ability to build successful relationships with team members and communicate effectively both 1-on-1 and in groups • The ability to work with care coordinators to direct care in the home through tele-assist capabilities • Strong technology skills and ability to use telehealth platforms • Can thrive in a fast-growing, mission driven organization focused on using data to improve patient outcomes
