• About Us: One Medical is a primary care solution challenging the industry status quo by making quality care more affordable, accessible and enjoyable. • The Opportunity: We are seeking a full-time Virtual Nurse Practitioner to join our One Medical at Home, high-risk care team and our High Risk Care Management virtual team, supporting our patients in multiple markets (Washington and Arizona). • What you'll likely work on: Manage patients with multiple, complex conditions through tele-assist home visits and telephone/video interventions, complete thorough initial and ongoing follow up visits focusing on geriatrics and chronic condition management, collaborate with care teams, and participate in after-hours call pool. • What you’ll need: Must have an accredited NP program with national certification, 5+ years experience in primary care, and be licensed in Washington.
Requirements
• Completed an accredited NP program with a national certification (ANCC, AANP) in Family, Geriatrics, or Adult • 5+ years of experience as a Nurse Practitioner with at least 2 years of home based, in office or virtual complex care, Geriatric Primary Care, geriatrics or palliative care experience • Bilingual in Spanish and English highly preferred • Currently licensed in Washington & able/ willing to obtain licensure in the state of Arizona and additional States as requested • Must be based in PST time zone • Must be willing to flex between PT/MT with advanced notice • Experience with home-based care preferred, specifically in the Seattle metropolitan area • Experience with value-based care principles and risk adjustment • The ability to build successful relationships with team members and communicate effectively both 1-on-1 and in groups • The ability to work with care coordinators to direct care in the home through tele-assist capabilities • Strong technology skills and ability to use telehealth platforms • Can thrive in a fast-growing, mission driven organization focused on using data to improve patient outcomes
