•About Us: One Medical is a primary care solution challenging the industry status quo by making quality care more affordable, accessible and enjoyable. •We are seeking a full-time Virtual Nurse Practitioner to join our One Medical at Home, high-risk care team supporting patients in multiple markets. •The Virtual Nurse Practitioner will care for geriatric, frail, and clinically complex patients through a team partnership. •One Medical has a network of primary care practices where we take the time to know our patients as individuals and proactively provide care and support.

Requirements

•Completed an accredited NP program with a national certification (ANCC, AANP) in Family, Geriatrics, or Adult •5+ years of experience as a Nurse Practitioner with at least 2 years of home based, in office or virtual complex care, Geriatric Primary Care, geriatrics or palliative care experience •Bilingual in Spanish and English highly preferred •Currently licensed in Washington & able/ willing to obtain licensure in the state of Arizona and additional States as requested •Must be based in PST time zone •Must be willing to flex between PT/MT with advanced notice •Experience with home-based care preferred, specifically in the Seattle metropolitan area •Experience with value-based care principles and risk adjustment •The ability to build successful relationships with team members and communicate effectively both 1-on-1 and in groups •The ability to work with care coordinators to direct care in the home through tele-assist capabilities •Strong technology skills and ability to use telehealth platforms •Can thrive in a fast-growing, mission driven organization focused on using data to improve patient outcomes
