Description

• Prevention of readmission for high-risk members through telephonic and in-home visits. • Comprehensive health risk assessments to understand member care coordination needs. • Medication reconciliation, adherence, and education. • Review of discharge summary for adherence. • Disease education including self-management and symptoms to report. • Appointment scheduling and follow-up. • Referral to social workers for socioeconomic support. • Care coordination for necessary additional services. • Direct access to members for concerns during the program. • Care summaries provided to PCPs and health plans upon program conclusion.

Requirements

• Current unrestricted Registered Nurse license. • Must be licensed in state of residence. • Minimum 5 years clinical experience with at least 3 years managed care experience. • 2 – 4 years experience in Managed Care preferred. • Telephonic and In-Home coverage required. • Ability to manage time and resources efficiently. • Excellent organizational skills. • Clear verbal and written communication skills. • Critical thinking and decision-making abilities. • Ability to work independently with little to no supervision. • Proficient in Microsoft Word, Excel, Outlook, and PowerPoint. • Familiarity with videoconferencing systems such as WebEx. • Typing speed of at least 30 wpm preferred. • Ability to sit at a desk for long periods.
