Description

• Promote improvement of health outcomes through case management. • Assess, plan, implement, coordinate, monitor, and evaluate options and services. • Establish collaborative relationships with clients and healthcare providers. • Adhere to professional practice and quality assurance standards. • Participate in training initiatives and demonstrate continuing education.

Requirements

• Active unrestricted Registered Nurse (RN) license in state or territory of the United States. • Two years full-time equivalent of direct clinical care to the consumer. • Previous case management experience with the Medicare/Medicaid population is ideal. • Knowledge of community resources. • Excellent verbal and written communication and interpersonal skills. • Strong time management and organization skills. • Ability to prioritize the work and work independently.
