• Conduct clinical assessments and care coordination for cancer patients. • Monitor member health changes and provide education over the phone. • Build relationships with members focusing on trust and empathy. • Ensure members have access to medications and appointments. • Perform telephonic assessments and coordinate care plans.
• A Bachelor of Science Degree in Nursing (BSN). • An unrestricted Registered Nurse (RN) license. • OCN certification required at time of hire or a commitment to obtain within 2 years. • At least 5 years of nursing experience with 3 years in oncology nursing or case management. • Certified Case Manager (CCM) certification. • Ability to communicate effectively and empathetically with members.
