Description
• Supports community connection activities • Provides members with known community resources • Supports coordination of community outreach resources • Serves as support for members on inquiries • Documents and maintains all community resources • Provides assistance to the clinical team • Conducts non-clinical general health assessments • Conducts telephonic and/or in-person outreach
Requirements
• Requires a High School diploma or GED • Requires 1 – 2 years of related experience • Field based work experience highly preferred • Community health worker experience or similar highly preferred
