• Leads the community and resource team on connection activities • Provides ongoing support and experienced insight to members • Supports the coordination of community outreach resources available to members • Serves as a subject matter expert to support members on community and care resource inquiries • Utilizes advanced knowledge of community resource connection activities and resource opportunities • Conduct non-clinical general health assessments in order to refer members to appropriate services • Document and maintain all community resources
Requirements
• Requires a High School diploma or GED • Requires 4+ years of related experience
