• Help elevate patient care • Collaborate with members, providers, and community organizations • Conduct comprehensive evaluation of member’s needs/eligibility • Coordinate and implement assigned care plan activities • Consult with case managers, supervisors, and Medical Directors • Utilize negotiation skills and influencing/motivational interviewing skills

Requirements

• 2 years experience in behavioral health, social services or appropriate related field • Preferred: Case management and discharge planning experience • Preferred: Managed Care experience • Preferred: Bachelor's degree or non-licensed master level clinician in behavioral health or human services
