• Conducts routine care coordination, support, and education for members. • Helps implement projects and processes for Case Management. • Consults with case managers and medical directors using a holistic approach. • Identifies and escalates quality of care issues. • Demonstrates negotiation skills to secure necessary options and services. • Provides coaching and support for member engagement. • Assists members in healthcare decision-making. • Conducts comprehensive evaluations of referred members’ needs.

Requirements

• Must reside near Cumberland, Kentucky Region. • Ability to travel up to 5% of the time. • Up to 2 years work experience in Case Management, Healthcare, or Clinical experience. • Basic awareness of problem solving and decision making skills. • High School Diploma, GED, or equivalent exp. • Bachelor's degree preferred.
