• Elevate patient care as Case Management Coordinator • Collaborate with members, providers, and community organizations • Conduct comprehensive evaluation of referred members’ needs/eligibility • Coordinate, implement, and monitor assigned care plan activities • Consult with case managers and Medical Directors to overcome barriers • Utilize negotiation skills to secure necessary options and services • Provide coaching and support for ongoing independent medical choices • Document care in compliance with regulatory guidelines

Requirements

• 2 years experience in behavioral health or social services • Must have the ability to work 8:00 AM to 5:00 PM local time • 2 years of experience with Microsoft Office applications (Word, Excel, Outlook)
