📋 Description
• HarmonyCares is one of the nation’s largest home-based primary care practices. • Responsible for enrolling eligible patients into the HarmonyCares program. • Lead community outreach activities and build relationships with physician offices. • Conduct research on community provider practices and patient healthcare needs. • Outreach and conduct in-person visits to healthcare facilities. • Collect and analyze patient outcomes and experiences.
🎯 Requirements
• College degree or 5 years of progressive career growth • 3+ years of related experience in sales, account management, community outreach or patient engagement in a medical/healthcare industry • An understanding of value-based care and how it can impact patient outcomes • Proficiency in Microsoft Excel, Word, and PowerPoint, ability to generate Excel reports • A dynamic and engaging personality with excellent interpersonal, verbal and written communication skills • Detail oriented, with the ability to think on your feet, responsive with great follow-up • Compassion, an interest in people and a professional presentation • Curious and a continuous learner, a passion for solving problems with a get-it-done attitude • Bilingual in Spanish, Mandarin or other language a big plus (preferred) • Comfort with CRM and EMR tools a plus (preferred)
