Today, as a Public Benefit Corporation (PBC), MHN has expanded its mission and impact to new markets and is a nationally recognized leader in transforming care within the safety net and building healthier communities across the country. Our whole person model of care, powered by proprietary technology, enhances collaboration across primary care and community-based organizations, leading to better outcomes, lower costs, and reduced health disparities. • Medical Home Network (MHN) is seeking a highly skilled Director of Population Health to lead initiatives aimed at improving patient care, enhancing clinical outcomes, and optimizing value-based care (VBC) performance. This role will combine expertise in population health with operational leadership, working closely with cross-functional teams to drive meaningful change in healthcare delivery. As a key member of the leadership team, the Director of Population Health will work with Data Analytics (MDAT), Financial Planning & Analysis (FP&A), and Clinical Operations to identify opportunities for improving care and driving business performance.

🎯 Requirements

• Bachelor’s degree required. MBA strongly preferred. Consulting background a plus. • 3+ years of experience in population health operations and data analytics, with 5-7+ years in healthcare. • Proven experience working in a value-based care (VBC) delivery or enablement environment. • Strong business and analytical mindset with the ability to translate data into strategy and operational improvements. • Expertise in Excel and data visualization tools (Power BI or Tableau). • Ability to roll up your sleeves and drive outcomes in a fast-paced environment. • Ability to identify strategic opportunities, develop actionable plans, and execute effectively. • Experience collaborating with cross-functional teams, including data analytics, finance, and clinical operations. • Strong business acumen and problem-solving skills. • Consulting experience a plus, specifically direct operations within a healthcare system. • Management experience strongly preferred.
