• If you got into healthcare to make a difference, you’re in the right place. • Looking for a values-driven, mission-focused Director, Social Work who is passionate about working with seniors. • Provides direct oversight to our Social Work team and leads strategic development of innovative solutions for the organization. • Serves as a thought partner for the Chief Medical Officer and drives integration of social work expertise into the Rippl Care Models. • Collaborates with leaders to ensure strategic, operational and clinical quality goals are met or exceeded. • Responsible for supporting effective interdisciplinary team rounding, process improvement and policy development to meet patient needs.
• Master’s Degree in Social Work (MSW) required; LCSW preferred. • 8+ years of experience in healthcare social work, with at least 3 years in a leadership role. • Highly preferred dementia and/or palliative care experience • Experience working in and leading interdisciplinary care teams within a healthcare setting. • Strong understanding of care planning, social determinants of health, and patient advocacy. • Proven ability to develop and implement social work programs. • Exceptional leadership, communication, and team-building skills. • Knowledge of healthcare regulations and best practices related to social work and care coordination. • Ability to work collaboratively with medical, clinical, and operational teams.
