Description

• Maintains operational accountability for care coordination and case management. • Directs all care coordination activities ensuring delivery meets NCQA and CMS standards. • Plans, implements, and provides leadership for clinical operational processes and staff. • Collaborates with various leaders to manage quality of care and cost of care. • Evaluates care coordination programs against quality and utilization benchmarks.

Requirements

• 7+ years of clinical experience. • Minimum of 3 years' direct supervisory experience including responsibility for analyzing complex datasets. • 2+ years' experience in care coordination and case management operations. • Required licenses: BCBA, LCMFT, LCSW, LMFT, LMHP, LPC, OT, PT, RN, State Licensure.
