📋 Description

• Manage the day-to-day operations of the Support Broker program, ensuring services are delivered effectively and efficiently. • Lead the execution of new initiatives, policies, and procedures aimed at improving client outcomes and program efficiency. • Provide leadership, guidance, and support to the Support Broker team, fostering a positive and productive work environment. • Set clear performance expectations and conduct regular performance evaluations for team members. • Develop and implement program-wide quality control measures. • Assist with gathering and reviewing data for required monthly and quarterly reports. • Follow operational procedures as defined by Palco. • Conduct in-home visits as required by program guidelines. • Build and maintain strong relationships with key stakeholders, including clients, families, service providers, community organizations, and regulatory bodies. • Address and manage any crisis situations that may arise, ensuring quick resolution and minimal disruption to service delivery. • Other duties as assigned.

🎯 Requirements

• Experience as a patient navigator, community connector, community health worker, lay health advisor, health educator, resource consultant, support broker, or case manager preferred. • Experience supervising a small group of remote staff or leadership staff. • A minimum of 1 year working with special needs population. • Bachelor’s Degree preferred; experience can be considered in place of a degree.
